What the result means

The page asks this site's Worker, which reads CF-Connecting-IP. That is the address Cloudflare associated with the connection. A value you put in X-Forwarded-For is ignored, because the browser can set that header to anything. Country, region, city, time zone, network, and colo come from request.cf. Latitude and longitude, when present, are city-level estimates, not a GPS reading and not a street address. Mobile networks and VPNs often place the city somewhere else. Nothing is written to a log or a database. Refresh runs the same request again. If you are on a VPN or a carrier gateway, the address belongs to that exit, not to the router in your home.

Is this my home address?

No. The result is a public network address and a coarse place name. It does not include a street, a building, or a GPS coordinate from your phone.

Why is the city wrong?

The city is inferred from the network that owns the address. Mobile carriers, privacy relays, and VPNs often exit in another city. The label on the page says the city is approximate for that reason.

Why is there no street address?

IP geolocation databases do not know a house. This page only shows fields Cloudflare already attached to the request, and those fields stop at city or region level.
